Why the Details I Dictate Don’t Show Up Where I Expect (and How to Correct It)

💡Sometimes, Comprehend may place information in the wrong section or repeat certain details. That’s because it doesn’t always know where you want each piece of information to go unless it's been trained on your preferences!

Here are two ways to fix that and make sure your notes come out exactly how you want them:

How to Train Comprehend Using Your Edits

Comprehend can learn from the changes you make to your notes—so it gets smarter and more accurate with each session.

Follow These Steps:

1. Open your patient's note in your EMR
2. Open the Comprehend Chrome extension and click '+New Visit' or open a previously recorded visit
3. If you click '+ New Visit' make sure to click Record and Stop Record (this signals Comprehend to sync to the note you're next to)
4. Make edits in your EMR—delete, reword, reorganize, or format things however you want.
5. Once your edits are done, click the Learn button in the Comprehend extension.
6. That’s it! The AI will now remember your preferences for future notes.

💡 This helps Comprehend improve your future notes by learning from your edits:

✅ If you delete something, it won’t show up next time.
✅ If you add something, it will try to include it in future notes.
✅ If you edit format, it will learn your structure.

✏️ Option 2: Manually Customize the AI Instructions (Best for Advanced Users)

1. Open a note in your EMR.
2. Open the Comprehend extension.
3. Click ‘+ New Visit’ or select a previously recorded visit.
4. If you click '+ New Visit' make sure to click Record and Stop Record (this signals Comprehend to sync to the note you're next to)
5. Scroll down to see all the note sections Comprehend can fill out.
6. Click into the section you want to update (e.g., ‘Assessment’).
7. You’ll see an ‘Instructions’ box — this tells Comprehend how to write that section.
8. Edit the instructions however you'd like — just like giving directions to a real scribe.
9. You can tell it what tone to use, what to include, what to leave out, and how to format it.
10. Finally click the small gray 'Save' button on the bottom right of the instruction box

🛠 You can update instructions for any section, but for most users, using the ‘Learn’ button is the easiest and most effective way to teach Comprehend how you document.
