How does Comprehend work with Clinicient/ InsightGo?

Filling Out the Note:

  1. Comprehend connects directly to the note you’re viewing in Clinicient.2. It recognizes the visit type (e.g., Standard Note, Initial Evaluation, Progress ,etc.) and identifies which sections of the note can be filled in for you.
  2. It recognizes the visit type (e.g., Standard Note, Initial Evaluation, Progress ,etc.) and identifies which sections of the note can be filled in for you.
  3. The sections Comprehend will fill out are then displayed in the Comprehend extension for you to review.
  4. You can adjust note settings to choose which sections of your note the AI fills out.
Each visit type has different documentation requirements, so this ensures your note is written within the visit context.


⏺️ Recording Your Visit:

  1.  Open the Patient Chart
  • Start by opening your patient chart as you normally would, just like you're preparing to fill it out yourself.
You don’t need the patient’s chart open while recording. Just open it later when you’re ready to fill out the note.


  1. Open the Comprehend Extension
  • Click the puzzle icon at the top-right corner of your browser to access your extensions.
  • Select the Comprehend extension to open it.
Link to download the Chrome extension, if you do not have it yet.
  1.  Create a New Visit
  • Click the ‘+ New Visit’ button in the top-right corner of the extension.
  • This will create a brand-new recording 🎉
  1.  Comprehend will automatically name the visit (if you’re on an open note)
  • Once you click the ‘+ New Visit’ icon, the extension will scan the page **if you’re open to a note.**
  • It will look for:
    • The patient’s name
    • The note type (e.g., Initial Eval, Follow up)
    • The sections it can fill out.
  1.  Record Your Visit Details
  • Click the “Record” button to start recording.
  • Choose how to provide visit information:

Option #1 →

Record the entire visit live (up to 120 min of recording time)

Option #2 →

Record yourself summarizing what happened during the visit.


Or do both! The AI uses all information recorded to write the note.


  1. Finish Recording
  • 🛑  When you're done, click ‘Stop’.



🚨Important Reminder:

Make sure to record all the information you want included in the note!

Sometimes, details like your assessment of the patient or measurements may not be said out loud during the visit. If that happens, be sure to record yourself mentioning them separately so they’re captured in the note.

All you have to do is click ‘Resume’ and add more information to the visit.


  1.  AI Processes the Recording
  • The AI will take a 3-10 seconds to analyze your recording and process the information.
  • Wait until it says ‘AI Complete’ or ‘Transcription Complete’ before clicking the Comprehend button.
🔐Fun Fact: The recording is deleted immediately after we process it! Our system creates notes based on the transcript, not the live recording, ensuring your data stays secure.


  1. EMR Note Type
  • Comprehend looks at the note type in your EMR to make sure it writes the note appropriately for that visit type.
  • For example, longer and more details for evals vs more concise updates for daily notes
  • If Comprehend picks the wrong note type, you can always change it by selecting a different one.

  1. After your visit is recorded, click the Comprehend button to generate your note.
  • This button generates a note for all the sections that are currently open in your EMR, using the visit recording to fill them out.
  1. You’re done! Read over the generated note and submit 🥳
✅ Tip: After making any edits to your note—whether adding details, removing information, or adjusting the format—click the “Learn”

What if I don’t want the Comprehend to fill out certain sections of my note?

You can choose to Skip any section.

  • You can easily set certain sections to ‘skip’. This means Comprehend will not change or fill out that section. This setting is always saved for that visit type until you change it.


🚨 You will have different note settings for each visit type (Evaluation, Progress Note, Daily Note, etc.)


  1. Open a patient note in your EMR
  2. Open the Comprehend extension
  3. Click the ‘+ New Visit’ button
  4. Scroll down in the extension to find the section in your EMR that you want to skip
  5. Move the Toggle to the left to set that section to ‘Skip’
  6. That’s it! The AI will now skip that section every time, and the setting will stay unless you change it 🎉
💡 Comprehend can see both Summary and Detail fields.

Customizing your notes. How to train the AI to write notes like you would!

Comprehend can learn from your edits ✍🏽

  • By default, the AI creates a standard note. Follow the steps below to customize the AI output so it writes notes like you would 😊

Step 1:

  • After the note is generated, make any edits you'd like—add details, delete anything unnecessary, or change the formatting of the entire note.

Step 2:

  • Once you're happy with the note, click Learn.
  • This teaches the AI your preferences based on your changes to the original note.
  • Next time, your note will be written more like your edited version—so you won't need to repeat the same edits.
✅Be sure to click Learn after making changes in Web PT to help the AI improve over time! That’s how the system is able to learn from your edits.

Can Comprehend update my note, instead of writing a brand new note?

  • Yes! Comprehend can use your previous note as context when creating a new one. This way, it understands what was written before and can update that information for you.

How Can Comprehend See Previous Information?

  • 🚨To ensure Comprehend can access previous notes, make sure your EMR is set up to carry notes forward to the next visit.
  • This setup allows the system to reference past information for creating new notes.

Step by Step Guide:

  1. Open the note you want to update in Web PT.
  2. Open the Comprehend extension
  3. Click ‘+ New Visit’ or click open an old visit
  4. Scroll in the extension to find the section you’d like to update.
  • Screenshot to the right shows how I edited the ‘Objective’ section
  1. Click section name to open the drop down
  • This shows you the ‘instructions,’ which explain how the AI is using the transcript to generate content in this section.
  1. Copy and paste the instructions below, or come up with your own!
  • As long as you reference the previous note in your instructions, you can tell the AI to do anything you’d like.
  1. Click the small ‘save’ button at the bottom right of the instruction box
  • You’re done! 🎉

Example Instruction for Daily Notes

Subjective

Update the previous note with any changes in patient’s reported symptoms or issues since the last visit. Try and use the patient’s own words when applicable. Specifically when describing symptoms / pain. Do so in 2-4 sentences.

Objective | Update the previous visits note with any new exercises done, changes in sets, reps, or weight.

If there are no changes, do not update the previous note. |

**Assessment **

**Update the previous visits note and comment on any progress on goals. Include how the patient tolerated today’s treatment. **

Plan

Update the previous visits note with any changes or updates in the treatment plan, if mentioned. Note adherence to prescribed exercises.

Goals

In the previous visits note, please comment on the patient’s progress towards their goals listed. Add a comment on progress and then today’s date at the beginning of the comment. Make sure it’s easy for the clinician to see progress over time.

What do the ‘Sync’ and ‘Comprehend’ buttons do?

Sync:

  • The 'Sync' button tells Comprehend to look at the note next to it and pull in the sections it can help complete into the Chrome extension.

🚨You don’t need to use the Sync button to generate a note!

The Sync button simply shows you which fields Comprehend could fill in on the current page.

To actually generate and fill out the note, just click ‘Comprehend’ on the page you’re working on, no need to click Sync first.


Comprehend Button:

  • The ‘Comprehend’ button generates the entire note for all of the detected sections that are open on the note you’re next to!

🔄 What do the refresh buttons do?

  • The Refresh button generates the note for that specific section only.
  • While the ‘Comprehend’ button generates a note for all the sections.

Refresh Icon Colors

What it means

GREEN

Comprehend successfully generated a complete note that followed the note setting instructions.

YELLOW 

If the icon is yellow, it means Comprehend was able to generate a complete note, but some of the information it looked for wasn’t included in the recording. This has nothing to do with your compliance score!

RED

Comprehend error, please contact support@comprehendpt.ai
