How does Comprehend work with Empower?

Filling Out the Note:

Comprehend connects directly to the note you’re viewing in Empower.
It recognizes the visit type (e.g., Initial Evaluation, Daily Note) and identifies which sections of the note can be filled in for you.

*Each visit type has different documentation requirements, so this ensures your note is written in the correctly

The available sections are then displayed in the Comprehend extension for you to review.
Comprehend button will on fill out the sections of the note that are applicable to your visit you recorded 🎉

How to Use the Comprehend Extension: Your Personal Medical Scribe!

Think of the Comprehend extension as your personal medical scribe! When open, it scans the page you're working on to see what it can fill out for you.

Comprehend works in three easy steps:

⏺️ Record your visit – either during the appointment or by summarizing it afterward.
📝 Generate your note – Comprehend will fill it directly into WebPT.
✅ Customize note output - Choose what sections get updated and train the AI to sound more like you!

Follow the steps below

Step-by-Step Guide to Getting Started:

Open Your Patient’s Chart

-Start by opening your patient chart as you normally would, just like you're preparing to fill it out.

**Open the Comprehend Extension

-Click the puzzle icon at the top-right corner of your browser to access your extensions.

-Select the Comprehend extension to open it.**

Create a New Visit

-Click the ‘+ New Visit’ button in the top-right corner of the extension.

-This will create a brand-new visit recording 🎉

4. Let Comprehend Scan the Page

Once you click the ‘+ New Visit’ icon, the extension will scan the page.
It will look for:
- The patient’s name
- The note type (e.g., Initial Eval, PT SOAP)
- The sections it can fill out.

5. Record Your Visit Details

Choose how to provide visit information:
- Record the entire visit live (up to 120 min of recording time)
- Record yourself summarizing what happened during the visit.
- Or do both!
Click the “Record” button to start recording.

6. Finish Recording

🛑 When you're done, click ‘Stop’.

Important Reminder:

Make sure to record all the information you want included in the note!

Sometimes, details like your assessment of the patient or measurements may not be said out loud during the visit. If that happens, be sure to record yourself mentioning them separately so they’re captured in the note.

All you have to do is Resume the recording and add more information to the visit.

7. AI Processes the Recording

The AI will take a 3-10 seconds to analyze your recording and process the information.

Fun Fact: The recording is deleted immediately after we process the transcript! Our system creates notes based on the transcript, not the live recording, ensuring your data stays secure.

8. EMR Note Type

Comprehend will automatically choose the right note setting based on the visit type (Daily Note, Eval, Progress Note etc.)

1. After your visit is recorded, click the Comprehend button to generate your note!

This button generates a note for all the sections that are currently open in your EMR, using the visit recording to fill them out.

Comprehend can only fill out what is visible on your screen—if you can't see it, we can't fill it out!

2. If your note has multiple pages, make sure to click into the specific section you want to work on before clicking ‘Comprehend’.

Continue to click and Comprehend each section until all sections are filled out.

3. You’re done! 🥳

What should I do if I want to leave certain section(s) of my note blank?

Open a patient note in the EMR
Open the Comprehend extension
Click the ‘+ New Visit’ button
Scroll down in the extension to find the section in your EMR that you want to skip
Move the Toggle to the left to set that section to ‘Skip’
That’s it! The AI will now skip that section every time, and the setting will stay unless you change it 🎉

Response Option	Outcome
Overwrite (default)	Erase anything that was previously in that field and input Comprehend PT’s generated note
Skip	Do not change anything in that field.
Add-on	Keep any text that was previously in that field. Have Comprehend’s generated note go underneath.

Customizing your notes. How to train the AI to write notes like you would!

✅ Learn from Edits! (RECOMMENDED)

Comprehend can learn from your edits ✍🏽

By default, the AI creates a standard note if you haven't uploaded your own examples yet. Follow the steps below to customize the AI output so it writes notes like you would 😊

Step 1:

After the note is generated, make any edits you'd like—add details, delete anything unnecessary, or change the formatting.

Step 2:

Once you're happy with the note, click Learn.
This teaches the AI your preferences based on your changes to the original note.
Next time, your note will be written more like your edited version—so you won't need to repeat the same edits.

Be sure to click Learn after making changes in Empower to help the AI improve over time! That’s how the system is able to learn from your edits.

Can Comprehend use my previous visit's note to help write a new one?

-Yes! Comprehend can use your previous visit note to help create new ones. This feature allows the system to carry over relevant details, saving you time and ensuring consistency in your documentation.

How Can Comprehend See Previous Information?

** - 🚨To ensure Comprehend can access previous notes, make sure your EMR is set up to carry notes forward to the next visit.

This setup allows the system to reference past information for creating new notes.**

Example Instructions you can use to update to previous visit’s note:

These are only examples, feel free to customize however you want 😊

Section	Example Instruction for Daily Notes
Subjective	Update the previous note with any changes in patient’s reported symptoms or issues since the last visit. Try and use the patient’s own words when applicable. Do so in 2-3 sentences.
Objective	Update the previous visits note with any new exercises done, or changes in sets, reps, and weight. If there are no changes, do not update the previous note.
Assessment	Update the previous visits note and comment on any progress on goals. Include how the patient tolerated today’s treatment.
Plan	Update the previous visits note with any changes or updates in the treatment plan, if mentioned. Note adherence to prescribed exercises.
Goals	In the previous vists note, please comment on the patient’s progress towards their goals listed. Add a comment on progress and then today’s date at the beginning of the comment. Make sure it’s easy for the clinician to see progress over time.

What do the ‘Sync’ and ‘Comprehend’ buttons do?

Sync:

The 'Sync' button tells Comprehend to look at the note next to it and pull in the sections it can help complete into the Chrome extension (refer to screenshot above).

▶️Demo: How to Use the Sync Button

You don’t need to use the Sync button to generate a note!

The Sync button simply shows you which fields Comprehend could fill in on the current page.

To actually generate and fill out the note, just click ‘Comprehend’ on the page you’re working on, no need to click Sync first.

Comprehend Button:

The ‘Comprehend’ button generates the entire note for all of the detected sections that are open on the note you’re next to!

What does the refresh button do?

The Refresh button generates the note for that specific section only.
While the ‘Comprehend’ button generates a note for all the sections

Refresh Icon Colors	What it means
GREEN	All information Comprehend was looking for was found in the recording.
YELLOW	If the icon is yellow, some of the information it looked for wasn’t included in the recording.
RED	Comprehend error, please contact support@comprehendpt.ai
