The Compliance feature in Comprehend helps ensure your documentation meets clinic and regulatory standards. It allows you to:


  1. Follow the preloaded compliance rules that follow CMS guidelines 
  2. Customize compliance rules based on your clinic’s needs
  3. Track performance through analytics
  4. Understand scoring to improve documentation quality in real time!


Using Preset Compliance Rules to Follow CMS Guidelines

Comprehend comes preloaded with compliance rules aligned with CMS guidelines to help ensure your notes meet required standards.

How It Works

  • The compliance feature automatically reviews your note for required elements based on CMS guidelines.
  • Each note is evaluated against preset rules.

Compliance Scoring

After analysis, each rule is assigned a status:

  • A = Met – The requirement has been fully satisfied
  • B = Partially Met – Some elements are present, but additional details are needed
  • F = Not Met – The requirement is missing

The system also provides suggestions to help improve compliance and strengthen your documentation.

Improving Your Score

  1. Review the feedback and suggestions provided for each rule.
  2. Add the recommended information to your recording or note.
  3. Run Comprehend again to re-evaluate your note.
  4. Continue refining until all rules are fully met and your compliance score improves.

This process helps ensure your notes are complete, accurate, and aligned with CMS standards.

How to Access

  1. Open Comprehend.
  2. Go to Admin Settings!


  1. Select Compliance Guidelines.


How to Edit Guidelines

  • Click View and Edit under CMS Guidelines.


  • Update existing rules or requirements!



Common Customizations

  • Mark as required if needed
  • Apply only to relevant note types
  • Add new categories, Define required elements (e.g., current, best, worst pain), Assign to specific note types!


Apply to Multiple Note Types

  • Use the dropdown to apply rules across Initial, Daily, or Progress notes.


Viewing Compliance Analytics

Admins can utilize the Analytics Dashboard to track team performance and pinpoint areas for improvement.

How to Access Analytics

  1. Open Comprehend.
  2. Go to Admin Settings.


  1. Click View Analytics.


Generate Reports
  1. Select a date range.
  2. Click Generate Analytics.
  3. Filter Your Data
  • Filter by user, role, or labels (e.g., PT, PTA).
  • Clear filters to view all users.


  1. Click on a user to see:
  • Average compliance score
  • Visit count
  • Detailed feedback


  1. Drill Into Notes

Open individual notes to see specific compliance details.

  • A – Met: Meets all compliance requirements.
  • B – Partially Met: Meets some requirements, but has missing or unclear elements.
  • F – Not Met: Does not meet requirements and is missing key elements.


  1. Share Reports
  • Use the Print option to export and share reports with your team.

Each note is graded based on how well it meets your compliance rules.


Scoring System

Each note is graded based on how well it meets your compliance rules.

How scoring works

  • Each rule is scored individually and contributes to a total of 100 points.
  • The number of rules determines how much each rule is worth.

Example: If there are 10 rules, each rule is worth 10 points.


Rule scoring

  • Fully met: Full points
  • Partially met: Half points
  • Not met: 0 points

Final score

  • Add up the points from all rules to get the note’s total score (out of 100).

Grading scale

  • A: 76–100
  • B: 50–75
  • F: 0–50



Important: Required Rules

If a rule is marked as Required and is not fully met → the note receives an F grade, regardless of other scores.



Best Practices

  • Clearly define which fields are required for your clinic.
  • Train staff on how compliance scoring works.
  • Regularly review analytics to spot trends.
  • Use labels to organize teams and improve reporting.
  • Continuously refine your guidelines based on feedback.
